Middle Park Health Director of Community Relations, Tiffany Freitag writes, “Another stellar member of the Middle Park Health Rehab team deserves recognition! Jacob Bauer, PT, DPT, has been awarded the Colorado Hospital Association’s Shining Star award for his positive impact to Colorado health care. In 2018 Jake was responsible for the creation of the MPH concussion management program in two local area high schools, West Grand and North Park. This included working with school officials and hospital staff to develop annual IMPACT testing, return to play guidelines, and education to coaches. In 2018, MPH was able to offer preseason IMPACT testing to all participating student-athletes from WGHS and NPHS. Jake also helped attract and hire an athletic trainer for Middle Park High School in 2018 and was involved in the start-up of the Wellness Center in Kremmling.

All of this in addition to his outpatient and inpatient clinical duties. Only twelve winners are awarded during each program year for the entire state. Jake was surprised by two representatives (one from Colorado Hospital Association and another from American Sentinel University) at our monthly rehab staff meeting with a delicious bundt cake,$100, and an invitation to an annual banquet with an opportunity to win a full-tuition scholarship from American Sentinel University. Way to go! Thanks for your contribution to the community Jake!”
