Double-Fatal Head-On Crash

On Thursday, July 18, 2019, at approximately 5:14 pm, the Colorado State Patrol responded to a two-car head-on crash on CO Highway 40 near milepost 186. This location is approximately 2 miles east of Kremmling, in Grand County, Colorado.

A 1998 white Jeep Cherokee driven by an unidentified female, was eastbound on Highway 40, drove off the right side of the road, traveled back onto the roadway and lost control, colliding head on with a 1999 Dodge Ram 3500 pickup driven by Ignacio Hernandez of Kremmling. The unidentified female driver was killed instantly, as well as the passenger, Nicholas Polashek of Blackhawk, CO. Hernandez, and Angel Mendez were transported by life flight with serious
injuries to St Anthony’s Hospital in Denver. Fernando Perez, the third passenger in the Dodge

Pickup had minor injuries and was not transported.

The crash is currently under investigation by the Colorado State Patrol. Speed and impairment are suspected as contributing factors in the crash. The roadway was dry at the time of the crash and no adverse weather conditions were noted
in the immediate area.

Identification of the female driver is pending notification of next of kin.
